In order to rollover an existing IRA account to FXDD, all clients must complete and submit following documents to the Custodian Firm:
1. Main Account Application (PDF). Please make sure that you IRA account number is indicate on Page 2.
2. Account Agreement, Risk Disclosure and Trading Rules. Sign and return Signature Page 28 only. This includes all of the rules and regulations you are to follow as a client of FXDD.
3. Additional Risk Disclosure Form Completion of this form is required if under the age of 21 or over the age of 65; if your income is less than $25,000;if you do not have at least six months of futures, securities, or foreign exchange investment or trading experience; if you are using retirement funds for trading capital. Under any of the above circumstances completion and signing of this form is required in order to be considered for a FXDD trading account.
*** Please make sure that last page of the Application and Customer Agreement Signature Page 26 are also stamped and signed by the Custodial Company.
FXDD and Millennium Trust and Entrust Administration are not related entities.
Once your account is opened at any of the above firms, the customer must complete all of FXDD's opening applications and documents and submit these documents to your custodial firm. The custodian will make a few changes on your application (e.g., such as change the account from being in your name to Entrust Admin FBO __). The custodial firm, upon completion of the application, will forward all paperwork to FXDD. Once the application is received by FXDD, the customer's regular account will be opened.No direct deposits or withdrawals may be issued in your trading accounts. All funds must be transferred by your custodial firm. Specific IRA questions should be addressed to the custodial firm.
